Service Description
Mental Health Skill Building Qualified Mental Health Professional (QMHP) Position Description: Under the supervision of the Clinical Director, the "Qualified Mental Health Professional-Adult" or "QMHP-A and theQualified Mental Health Professional Trainee will provide direct Mental Health Support Services (MHSS) that implement a service plan, which provides structure, support, and services to clients. These services are provided in accordance with agency policies. Position Qualifications: The Mental Health Skill Building Counselor must be registered with the Board of Counselor as a Qualified Mental Health Professional-Adult (QMHP-A) or a Qualified Mental Health Professional Trainee (QMHP-T). Position Responsibilities: Provides direct, Mental Health Support Services (MHSS) to identified client (Ensure that the required LMHP documents have been gathered and completed to ensure that the Qualified Mental Health Professional-Adult (QMHP-A), and Qualified Mental Health Professional Trainee (QMHP-T), and all involved parties can participate in the preparation of treatment plan and updates. Assists in planning of services. Provides client with day-to-day skills/education as well as support to assist client in meeting their day-to-day obligations. Models and teaches problem-solving techniques. Provides a variety of interventions in client’s home and community as appropriate and necessary. These interventions may be behavioral, financial and/or from a mental health skill building Provides linkage to community-based services and support; provides advocacy and support for the client. Coordinates scheduling of appointments, as needed. Coordinates services, as needed and Link Crisis plan to appropriate mental health provider Is responsible for the proper and timely documentation of all information required by the Department of Behavioral Health & Developmental Services (DBHDS) and HARMONY1, LLC Mental Health Services, Maintains regular and ongoing contact with referring agencies, as well as other agencies involved in client Actively coordinates and plans discharge, and aftercare plans with referring agency. Attends interagency meetings, as necessary. Attends and participates in team meetings. Attends and participates in orientation, in-service training, and conferences as scheduled. Attending supervision. Performs other duties as assigned. Driver's License w/current inspection and vehicle auto insurance acceptable driving record
